If you are asking for spousal or partner support as part of a domestic violence restraining order case:
1. In addition to domestic violence restraining order forms, fill out the following:
- Spousal, Partner, or Family Support Order Attachment (Form FL-343)

- Income and Expense Declaration (Form FL-150)

See the instructions for this form.
2. Forms for the Respondent: (usually served on him or her by the petitioner)
3. Proof of Service forms:
- Proof of Service (In Person) (Form DV-200)

NOTE: If the petitioner is only serving Income and Expense forms, he or she can use Service by Mail.The respondent can also use Service by Mail when serving his or her Answer.
- Proof of Service by Mail (Form DV-250)

4. After the hearing you MAY also need these forms:
- Findings and Order After Hearing (Form FL-340)

- Spousal, Partner, or Family Support Order Attachment (Form FL-343)
